Select the type of referral and fill in the necessary data. Once completed, press save followed by submit to successfully send the referral to NEHR.<\/span><\/p><p><img loading=\"lazy\" decoding=\"async\" class=\"alignnone wp-image-16418 size-full\" src=\"https:\/\/www.medicare-asia.com\/wp-content\/uploads\/2024\/12\/Referral2.jpg\" alt=\"\" width=\"542\" height=\"690\" \/><\/p><p><span style=\"font-weight: 400;\">Next, if the patient have chronic diseases, head to the patient\u2019s Personal Info -&gt; Diagnosis and fill in their chronic diseases<\/span><\/p><p><img loading=\"lazy\" decoding=\"async\" class=\"alignnone wp-image-16414 size-large\" src=\"https:\/\/www.medicare-asia.com\/wp-content\/uploads\/2024\/12\/Chronic-Diagnosis-1024x548.jpg\" alt=\"\" width=\"1024\" height=\"548\" \/><\/p><p><span style=\"font-weight: 400;\">Once the data are filled in, head to the Health Plan tab -&gt; Care report Tab -&gt; Generate Care Report<\/span><\/p><p><img loading=\"lazy\" decoding=\"async\" class=\"alignnone wp-image-16410 size-large\" src=\"https:\/\/www.medicare-asia.com\/wp-content\/uploads\/2024\/12\/Care-Report-1-1024x432.jpg\" alt=\"\" width=\"1024\" height=\"432\" \/><\/p><p><span style=\"font-weight: 400;\">After clicking the \u201c+ Generate Care Report\u201d, you will be able to see this page with the following information:<\/span><\/p><ol><li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Patient information<\/span><\/li><li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Smoking habits from the health facts<\/span><\/li><li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Chronic Disease Management from the Patient Info -&gt; Diagnosis<\/span><\/li><li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Addition of external visits to be attached to the care report<\/span><\/li><li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Using in-house visit record to be attached to the care report<\/span><\/li><\/ol><p><img loading=\"lazy\" decoding=\"async\" class=\"alignnone wp-image-16411 size-large\" src=\"https:\/\/www.medicare-asia.com\/wp-content\/uploads\/2024\/12\/Care-Report-2-1024x547.jpg\" alt=\"\" width=\"1024\" height=\"547\" \/><\/p><p><span style=\"font-weight: 400;\">After clicking on 4. External Visits or 5. Using in-house Visit Record, You will be able to see the following information:<\/span><\/p><ol><li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Patient BMI from the Health Measurement<\/span><\/li><li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Any referral created<\/span><\/li><li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Additional Diagnosis and complications<\/span><\/li><li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Screening information<\/span><\/li><li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Vaccination information<\/span><\/li><\/ol><p><img loading=\"lazy\" decoding=\"async\" class=\"alignnone wp-image-16413 size-large\" src=\"https:\/\/www.medicare-asia.com\/wp-content\/uploads\/2024\/12\/Care-Reprot-3-1024x444.jpg\" alt=\"\" width=\"1024\" height=\"444\" \/><\/p><p><span style=\"font-weight: 400;\">Scrolling down, there are several measurements that you may key in<\/span><\/p><p><img loading=\"lazy\" decoding=\"async\" class=\"alignnone wp-image-16419 size-large\" src=\"https:\/\/www.medicare-asia.com\/wp-content\/uploads\/2024\/12\/Care-report-measurement-1024x340.jpg\" alt=\"\" width=\"1024\" height=\"340\" \/><\/p><p><img loading=\"lazy\" decoding=\"async\" class=\"alignnone wp-image-16420 size-large\" src=\"https:\/\/www.medicare-asia.com\/wp-content\/uploads\/2024\/12\/Measurement-2-1024x327.jpg\" alt=\"\" width=\"1024\" height=\"327\" \/><\/p><p><span style=\"font-weight: 400;\">After filling in the measurements and pressing save, you will be brought back to the Care report generation page. At this point, you will notice that the visit you have filled in is automatically selected for attachment to the care report.\u00a0<\/span><\/p><p><span style=\"font-weight: 400;\">At this point, you may choose to select more visits to be attached to the care report if needed. Do make sure that the visits that are needed to be attached, has the green \u201ccheck\u201d icon prior to submission.<\/span><\/p><p><span style=\"font-weight: 400;\">Otherwise,\u00a0 please click on \u201csave\u201d followed by submit.<\/span><\/p><p><img loading=\"lazy\" decoding=\"async\" class=\"alignnone wp-image-16412 size-large\" src=\"https:\/\/www.medicare-asia.com\/wp-content\/uploads\/2024\/12\/Care-report-4-1024x478.jpg\" alt=\"\" width=\"1024\" height=\"478\" \/><\/p><p>After clicking on Submit, You should return be able to see the status change from Submitted.
